Code Ann. § 17-95-908","heading":"Notification of intent to practice","body":"(a) (1) Before initiating practice, a graduate registered physician licensed in this state must submit on forms approved by the Arkansas State Medical Board notification of an intent to practice. (2) The notification shall include: (A) The name, business address, email address, and telephone number of the supervising physician; and (B) The name, business address, and telephone number of the graduate registered physician. (b) A graduate registered physician shall notify the board of any changes or additions in supervising physicians within ten (10) calendar days.
