Code Ann. § 4-94-106","heading":"Registration statement","body":"(a) Any person, corporation, partnership, association, or group intending to open or operate a health spa shall file an annual registration statement with the office of the Secretary of State prior to the sale of any contracts for health spa services. (b) The registration statement shall contain: (1) The name and address of the health spa; (2) The names and addresses of the officers, directors, and stockholders of the health spa and its parent corporation, if such entity exists; and (3) The types of available facilities.
