An applicant for licensure as an embalmer or funeral director shall submit a completed application on a form prescribed by the department. The application shall be subscribed under oath and shall be accompanied by:\n\n1. Any educational, professional and employment information required by the department in its rules.\n\n2. A completed fingerprint card and the prescribed fingerprint background check fee.\n\n3. Any other information required by the department.\n\n4. All applicable fees pursuant to section 32-1309.\n\nB.
