The board by formal vote at its annual meeting shall establish fees not to exceed the following amounts:\n\n1. Initial application for certification for certified registered nurse anesthetist, registered nurse practitioner and clinical nurse specialist in specialty areas, $150.\n\n2. Initial application for school nurse certification, $75.\n\n3. Initial application for license as a registered nurse, $150.\n\n4. Initial application for license as a practical nurse, $150.\n\n5. Application for reissuance of a registered or practical nursing license, $150.\n\n6. Application for renewal of a registered nurse or a practical nurse license before expiration, $160.\n\n7. Application for renewal of license after expiration, $160, plus a late fee of $50 for each month the license is lapsed, but not to exceed a total of $200.\n\n8. Application for renewal of a school nurse certificate, $50.\n\n9. Application for temporary registered nurse, practical nurse or licensed nursing assistant license, $50.\n\n10. Retaking the registered nurse or practical nurse examination, $100.\n\n11. Issuing a license to an applicant to become a licensed nursing assistant or licensed health aide, $50.\n\n12. Issuing a license to a licensed nursing assistant or licensed health aide applicant for renewal, $50.\n\n13. Application for renewal of a licensed nursing assistant or licensed health aide license after its expiration, $25 for each year it is expired, not to exceed a total of $100.\n\n14. Issuing a duplicate license or certificate, $25.\n\n15. Copying a nursing program transcript, $25.\n\n16. Verification to another state or country of licensure for endorsement, certification for advanced practice or licensed nursing assistant licensure, $50.\n\n17. Providing verification to an applicant for licensure or for licensed nursing assistant licensure by endorsement, $50.\n\n18. Application to prescribe and dispense medication and application to prescribe medication, $150.\n\n19. Application for renewal of prescribing and dispensing medication privileges before expiration and application for renewal of prescribing medication privileges before expiration, $20.\n\n20. Application for renewal of prescribing and dispensing medication privileges after expiration and application for renewal of prescribing medication privileges after expiration, $35.\n\n21. Issuing an inactive license, $50.\n\n22. Writing the national council licensing examination for the first time, $150.\n\n23. Sale of publications prepared by the board, $50.\n\n24. Providing notary services, $2, or as allowed under section 41-316.\n\n25. Copying records, documents, letters, minutes, applications and files, $.50 a page.\n\n26. Processing fingerprint cards, $50.\n\n27. Registration for board seminars, $100.\n\n28. Failing to notify the board of a change of address pursuant to section 32-1609, $25.\n\nB.
