On request of any person, a health profession regulatory board must provide the following information to that person:\n\n1. A copy of the minutes of any specified board meeting.\n\n2. A copy of a board action concerning a person regulated by the board.\n\n3. A copy of the final adjudication of a complaint against a person regulated by the board. For the purposes of this paragraph, final adjudication of a complaint does not include any complaint that was dismissed or terminated more than five years before the request was submitted.\n\n4. The name and primary practice address of a person regulated by the board.\n\nB.
