A private school shall submit a copy of its official catalog to the board for board approval.\n\nB. A private school catalog shall contain the following:\n\n1. Name and address of the school.\n\n2. Date of publication.\n\n3. Admission requirements and procedures used by the school.\n\n4. Number of hours of training required for licensure.\n\n5. A brief outline of the curriculum offered by the school.\n\n6. A description of the school's general physical facilities and equipment.\n\n7. Policies relating to tardiness, absences, make-up work, conduct, termination and other rules of the school.\n\n8. The grading system, including a definition of credit units if any.\n\n9.
