{"data":{"id":"us-ca/ins-15010","jurisdiction":"us-ca","citation":"INS § 15010","heading":"","body":"An application shall be verified and shall include all of the following:\n(a) The full name and business address of the applicant.\n(b) The name under which the applicant intends to do business.\n(c) A statement as to the general nature of the business in which the applicant intends to engage.\n(d) If the applicant is a person other than an individual, the full name and resident address of each of its partners, officers, and directors.\n(e) Two photographs, not older than six months, of the applicant, of a type prescribed by the commissioner, and one classifiable set of his or her fingerprints, to be sent to a live scan fingerprint provider as directed by the department, if fingerprints are not submitted in person with a live scan fingerprinting service provider certified by the Department of Justice.\n(f) A verified statement of his or her experience qualifications.\n(g) Other information, evidence, statements, or documents as may be required by the commissioner.","path":["Insurance Code - INS","DIVISION 5.
