{"data":{"id":"us-co/c.r.s.-12-285-111","jurisdiction":"us-co","citation":"C.R.S. § 12-285-111","heading":"Provisional license - fee.","body":"(1) The board may issue a provisional license to practice as a physical therapist to a person who:\n\n(a) Submits an application and pays a fee as determined by the director; and\n\n(b) Successfully completes a physical therapy program that meets the educational requirements in section 12-285-110 (1)(a).\n\n(2) A person who holds a provisional license may only practice under the supervision of a physical therapist actively licensed in this state.\n\n(3) A provisional license issued pursuant to this section expires no later than one hundred twenty days after the date it was issued.
