{"data":{"id":"us-co/c.r.s.-26-6-908","jurisdiction":"us-co","citation":"C.R.S. § 26-6-908","heading":"Application forms - criminal sanctions for perjury.","body":"(1) (a) (I) All applications for the licensure of a child placement agency, a residential or day treatment child care facility, or an applied behavior analysis clinic or the certification of a foster care home or a kinship foster care home pursuant to this part 9 must include the notice to the applicant set forth in subsection (1)(b) of this section.\n\n(II) Every application used in the state of Colorado for employment with a facility or agency must include the notice to the applicant that is set forth in subsection (1)(b) of this section.\n\n(b) Each application described in subsection (1)(a) of this section must contain the following notice to the applicant:\n\nAny applicant who knowingly or willfully makes a false statement of any material fact or thing in this application commits perjury in the second degree as defined in section 18-8-503, Colorado Revised Statutes, and, upon conviction thereof, shall be punished accordingly.\n\n(2) An individual applying for the licensure of a facility, agency, or applied behavior analysis clinic or the certification of a foster care home or kinship foster care home pursuant to this part 9, or an individual applying to work at a facility, agency, or clinic as an employee, who knowingly or willfully makes a false statement of any material fact or thing in the application commits perjury in the second degree as defined in section 18-8-503 and, upon conviction, shall be punished accordingly.\n\n(3) Every application for licensure or certification as a foster care home or kinship foster care home must provide notice to the applicant that the applicant may be subject to immediate revocation of licensure or certification or other negative licensing action as set forth in this section (3) and section 26-6-913 and as described by rule of the state board.","path":["Title 26 - HUMAN SERVICES CODE","Article 6 - Child Care Centers","Part 9 - FOSTER CARE, KINSHIP FOSTER CARE, RESIDENTIAL, DAY TREATMENT, AND CHILD PLACEMENT AGENCY LICENSING AND CERTIFICATION"],"source_url":"https://olls.info/crs/crs2026-title-26.htm","current_through":"Colorado Revised Statutes 2026","vintage":"","retrieved_at":"2026-09-14T18:37:45Z","sha256":"a26af6857a75474c4d7e84b7cb36f7dbc5548ee0c2114d88cf6a42f90da82b36","source_id":"us-co","stale":false,"prev":"us-co/c.r.s.-26-6-907","next":"us-co/c.r.s.-26-6-909"},"notice":"GroundRules: Original legal text.
