Code § 21-2221.06","heading":"Revocation of a MOST Form.","body":"(a)\nA patient or the patient's authorized representative may revoke a MOST Form at any time by:\n(1)\nDirecting the authorized health care professional who issued the MOST Form to cancel the MOST Form; or\n(2)\nCommunicating the patient's or the patient's authorized representative's intent to revoke the MOST Form to the treating EMS personnel or health care professional.\n\n(b)\nIf a patient or the patient's authorized representative revokes a MOST Form pursuant to subsection (a)(2) of this section, the treating EMS personnel or health care professional shall record the circumstances in which the MOST Form was revoked.","path":["Title 21.
