Code § 44-103.02","heading":"Initial ALR licensure.","body":"(a)\nApplications for licensure shall be made in writing on a form or forms prescribed by the Mayor at least 60 days prior to the date needed.\n\n(b)\nAn applicant for ALR licensure shall pay a licensure fee as determined by the Mayor.\n\n(c)\nAn ALR license issued by the Mayor shall state the effective date and expiration date of the license and maximum number of residents allowed to reside in the ALR.\n\n(d)\nAn application for an ALR license shall include the following information:\n(1)\nIdentification of the owner and documentation supporting the fact that the ALR is owned or otherwise under the control of the applicant;\n(2)\nIdentification of the ALA and information concerning the ALA’s qualifications;\n(3)\nCriminal background check information pursuant to subchapter II of Chapter 5 of this title;\n(4)\nDocumentation and explanation of any prior denial, suspension, or revocation of license to provide care to third parties;\n(5)\nLocation of the ALR;\n(6)\nStatement of program;\n(7)\nProof of solvency;\n(8)\nProof of insurance coverage;\n(9)\nStatement of services to be offered;\n(10)\nMaximum number of residents planned;\n(11)\nVerification that the real property where the ALR is located is owned, leased, or otherwise under the control of the applicant; and\n(12)\nStructure of applicant’s organization and names of board members and officers.\n\n(e)\n(1)\nThe Mayor shall conduct an initial pre-licensure inspection of the premises of the ALR and of its records.\n(2)\nAn applicant for licensure shall provide the following information at the time of the pre-licensure inspection:\n(A)\nCertificate of occupancy;\n(B)\nDisaster plan;\n(C)\nStaffing plan;\n(D)\nResident funds management system;\n(E)\nMedication management system;\n(F)\nIndividual Service Plan policy and procedures;\n(G)\nAdmission, transfer, and discharge policies;\n(H)\nResident agreements, both financial and nonfinancial;\n(I)\nLocation of the ALR;\n(J)\nMaximum number of residents to be served;\n(K)\nProgram statement;\n(L)\nProof of solvency; and\n(M)\nOther reasonably relevant information required by the Mayor.\n\n(f)\nBased on information obtained during the pre-licensure inspection required by subsection (e) of this section, the Mayor shall either approve the application unconditionally for 12 months or deny the application.\n\n(g)\nThe Mayor shall re-inspect an ALR within 6 months of the effective date of the initial licensure.","path":["Title 44.
