Code § 44-1051.04","heading":"Application for initial license.","body":"A nurse staffing agency shall submit to the Department, as part of the agency’s initial application for licensure:\n\n(1)\nThe business name of the agency;\n\n(2)\nThe addresses of the agency’s registered business office, operations headquarters, and District of Columbia operations headquarters;\n\n(3)\nThe telephone numbers of all offices listed in paragraph (2) of this subsection;\n\n(4)\nIf the agency is a corporate entity, the entity’s Certificate of Good Standing as a corporation;\n\n(5)\nThe name of the agency’s responsible party;\n\n(6)\nAny other information necessary to ensure compliance with the provisions of this chapter, as established by regulation; and\n\n(7)\nThe initial licensure fee as established by regulation.","path":["Title 44.
