C. § 1220","heading":"Definitions.","body":"As used in this subchapter:\n(1) “Health-care practitioner” means as defined in § 701 of this title.\n(2) “Health-care professional” means as defined in § 701 of this title.\n(3) “Informed consent” means the signing of a consent form by the patient or a person authorized to make health-care decision on behalf of the patient that satisfies all of the following conditions:\na. Is written in plain language.\nb. Is dated.\nc. Includes a description of the procedure to be performed.\nd. States if there will be a student or resident that will perform or be present at the patient’s examination.","path":["Title 16.
