The owner of an amusement ride shall apply for a permit to the office on a form furnished by the office providing such information as the office may require.\n(b) No such application shall be complete without including a certificate of inspection from a licensed inspector that the amusement ride meets all relevant provisions of this article and the standards and regulations adopted pursuant thereto.
