{"data":{"id":"us-gu/10-gca-12621","jurisdiction":"us-gu","citation":"10 GCA § 12621","heading":"Qualifications for Registration to Practice Telepharmacy Across Guam/State Lines.","body":"(a) An applicant applying for registration to engage in the practice of telepharmacy Across Guam/State Lines shall:\n(1) present to the Board proof of licensure in another jurisdiction and proof that such license is in good standing;\n(2) submit a written application in the form prescribed by the Board of Pharmacy;\n(3) pay the fee(s) specified by the Board of Pharmacy for issuance of the license; and\n(4) comply with all other requirements of the Board of Pharmacy.\n(b) Application.\n(1) The written application required under § 12617(a)(1) of this Act shall request of the applicant, at a minimum, the following information:\n(A) name, address and current pharmacist licensure information in all other jurisdictions, including jurisdiction(s) of licensure and license number(s);\n(B) name, address, phone number, and, if applicable, jurisdiction of licensure and license number of the site where the practice of telepharmacy will originate;\n(C) a statement of the scope of patient services that will be provided;\n(D) a description of the protocol or framework by which patient care will be provided, including any collaborative practice arrangements with other health care practitioners; and\n(E) a statement attesting that the applicant will abide by the pharmacy laws and regulations of the jurisdiction in which the patient is located.","path":["Title 10: Health and Safety","Division 1 - Public Health","Chapter 12: Medical Practices: Part 1: Articles 1 - 10","PART 1","ARTICLE 6: GUAM PHARMACY PRACTICE ACT"],"source_url":"https://col.guamcourts.gov/sites/default/files/10gc012-1_0.pdf","current_through":"P.L. 38-133 (June 4, 2026)","vintage":"","retrieved_at":"2026-09-27T02:54:48Z","sha256":"3aea00b438e39895de95dd2ffcefee0e90ab6f65dcc650561330e04396aea7f1","source_id":"us-gu","stale":false,"prev":"us-gu/10-gca-12620","next":"us-gu/10-gca-12622"},"notice":"GroundRules: Original legal text.
