{"data":{"id":"us-gu/10-gca-12806","jurisdiction":"us-gu","citation":"10 GCA § 12806","heading":"Responsibilities of Licensure Applicants.","body":"Each applicant shall be responsible for the following:\n(a) correspondence or communication with the Board;\n(b) submission of all required documents;\n(c) periodically contacting the Board concerning the status of the application;\n(d) informing the Board in writing of intent to withdraw an application prior to the remittance of the applicable fee; fees remitted to the Board are non-refundable;\n(e) informing the Board of any change in name or address (An applicant whose name has changed shall submit a certified copy evidencing the change to the Board. The Board upon receipt of such evidence and the required fee shall issue a duplicate license with the name change.
