{"data":{"id":"us-gu/5-gca-34121.1","jurisdiction":"us-gu","citation":"5 GCA § 34121.1","heading":"Information to be Included in Support Orders.","body":"(a) Every Court order for the support of a child issued or modified in Guam on or after the effective date of this legislation, must include:\n(1) the names, dates of birth and social security numbers of the parents of the child;\n(2) the name, date of birth and social security number of the child; and\n(3) the case identification number assigned by the Court.\n(b) Within ten (10) days after a tribunal issues an order for the support of a child, each party to the cause of action shall file with the Court that issued the order and the Department of Law, Family Division:\n(1) the party’s social security number;\n(2) the party’s residential and mailing addresses;\n(3) the party’s telephone number or a contact number;\n(4) the party’s motor vehicle operator’s license number; and\n(5) the name, address and telephone number of each party’s employer.
