A statement by the issuing authority that the arrest warrant was issued after a determination of probable cause to believe that a crime had been committed and that the demanded person committed the crime, together with a copy of the provisions of law defining the crime and fixing the penalty therefor;\n2. A certified copy of the indictment or information upon which the arrest warrant is based;\n3. A statement by the issuing authority that the arrest warrant was issued after a determination of probable cause to believe that the demanded person had violated any term of bail, probation, or an order arising out of a criminal proceeding; or\n4.
