A statement by the issuing authority that the arrest warrant was issued after a determination of probable cause to believe that a crime has been committed and the requested person committed the crime, together with a copy of the provisions of law defining the crime and fixing the penalty therefor;\n2. A certified copy of the indictment or information upon which the arrest warrant is based;\n3. A statement by the issuing authority that the warrant was issued after a determination of probable cause to believe that the requested person had violated any term of bail, probation, or other judicial order arising out of a criminal proceeding; or\n4.
