(a) A statewide aging and disability resource center may be established with sites in each county to streamline access to long-term supports and services by integrating the full range of long-term supports and services into a single, coordinated system.\n\n(b) The aging and disability resource center may be the single point of entry in every county where persons of all ages, incomes, and disabilities may access information in a person-centered manner on the full range of long-term supports and services options, including but not limited to:\n\n(1) Federal, state, and county revenue-funded programs and services including those funded by medicaid, medicare, the Older Americans Act, the Department of Veterans Affairs, and kupuna care;\n\n(2) A centralized application process for publicly funded long-term services and supports;\n\n(3) Privately administered programs and services;\n\n(4) Supports and services for persons with Alzheimer's disease and other related dementia;\n\n(5) Transportation services;\n\n(6) Housing options;\n\n(7) Elder rights protection;\n\n(8) Hospital and nursing home discharge planning and care transition;\n\n(9) Health, prevention, and wellness programs;\n\n(10) Support for grandparents raising grandchildren and other relatives age fifty-five years or older caring for children;\n\n(11) Informal and family caregiver support services; and\n\n(12) Community resources and services for individuals with disabilities.\n\n(c) The aging and disability resource centers shall target delivery of services to:\n\n(1) Persons sixty years of age and older;\n\n(2) Persons of any age with physical disabilities, severe mental illness, dementia, and developmental or intellectual disabilities;\n\n(3) Informal and family caregivers providing assistance to persons needing long-term supports and services;\n\n(4) Professionals seeking long-term supports and services on behalf of their clients; and\n\n(5) Persons planning for their future long-term supports and services needs.\n\n(d) The executive office on aging shall coordinate the implementation of the statewide aging and disability resource center.\n\n(e) The aging and disability resource center may be a function of each area agency on aging within the respective geographic service area.","path":["HI Code","Division 1","Title 20","Chapter 349"],"source_url":"http://www.capitol.hawaii.gov/hrscurrent/Vol07_Ch0346-0398/HRS0349/HRS_0349-0032.htm","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:11Z","sha256":"ba2a2ecfc9df30c50534a261b0dcc7955f8eff5759371555856ef1bf72f5ccf5","source_id":"us-hi","stale":false,"prev":"us-hi/haw.-rev.-stat.-349-3.2","next":"us-hi/haw.-rev.-stat.-349-4"},"notice":"GroundRules: Original legal text.
