Stat. § 481X-3","heading":"Registration.","body":"(a) Before conducting business in this State, a provider shall register with the commissioner, on a form prescribed by the commissioner, and shall pay to the commissioner a fee as provided under section 431:7-101.\n\n(b) Provider registration shall be updated annually and shall contain the following information:\n\n(1) The address of the principal office of the provider;\n\n(2) The name and address of the provider's agent for the service of process in this State, if other than the provider;\n\n(3) The identities of the provider's executive officer or officers directly responsible for the provider's service contract business;\n\n(4) The name, address, and telephone number of any administrators designated by the provider to be responsible for the administration of service contracts in this State ; and\n\n(5) A statement that the provider is in compliance with the financial responsibility requirements of section 481X-4 and that details how the provider intends to meet the requirements, and proof of compliance with the requirements.","path":["HI Code","Division 2","Title 26","Chapter 481X"],"source_url":"http://www.capitol.hawaii.gov/hrscurrent/Vol11_Ch0476-0490/HRS0481X/HRS_0481X-0003.htm","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:11Z","sha256":"713e47dd727b3ffb78699bcffd591184a9344156a6520f043087980af3e81fdc","source_id":"us-hi","stale":false,"prev":"us-hi/haw.-rev.-stat.-481x-2","next":"us-hi/haw.-rev.-stat.-481x-4"},"notice":"GroundRules: Original legal text.
