A written request for a cost-of-living alteration to the support order signed by the parent making the request.\n2. A statement signed by the nonrequesting parent agreeing to the cost-of-living alteration to the support order.\n3. A statement signed by each parent waiving that parent’s right to personal service and accepting service by regular mail.\n4.
