One hundred fifty dollars for a new application.\nb. One hundred fifty dollars for a reinstatement.\nc. Twenty-five dollars to change an address or name.\nd. Ten dollars for tariff updates.\ne. Twenty-five dollars for a duplicate permit or certificate.\n2. Changes in ownership of motor carrier permits require a new application and the new application fee of one hundred fifty dollars shall be assessed.\n3.
