First card issued to a youth\nfor whom the Department of Children\nand Family Services is legally responsible\nor a foster child upon turning the age of\n16 years old until he or she reaches\nthe age of 21 years old............. Original card issued to a committed\nperson upon release from the\nDepartment of Corrections, the\nDepartment of Juvenile Justice,\na Federal Bureau of Prisons\nfacility located in Illinois,\nor a county jail or a county\ndepartment of corrections . Limited-term Illinois Identification\nCard issued to a committed person\nupon release from the Department of\nCorrections, the Department of\nJuvenile Justice, a Federal Bureau\nof Prisons facility located in\nIllinois, or a county jail or a\ncounty department of corrections . No Fee\nt. Original card issued to a\nperson up to 14 days prior\nto or upon conditional release\nor absolute discharge from\nthe Department of Human Services....... No Fee\nu. Limited-term Illinois Identification\nCard issued to a person up to\n14 days prior to or upon\nconditional release or absolute discharge\nfrom the Department of Human Services.... No Fee\nAll fees collected under this Act shall be paid into the Road Fund of the State treasury, except that the following amounts shall be paid into the General Revenue Fund: (i) 80% of the fee for an original, renewal, or duplicate Illinois Identification Card issued on or after January 1, 2005; and (ii) 80% of the fee for a corrected Illinois Identification Card issued on or after January 1, 2005.\nAn individual, who resides in a veterans home or veterans hospital operated by the State or federal government, who makes an application for an Illinois Identification Card to be issued at no fee, must submit, along with the application, an affirmation by the applicant on a form provided by the Secretary of State, that such person resides in a veterans home or veterans hospital operated by the State or federal government.\nThe application of a homeless individual for an Illinois Identification Card to be issued at no fee must be accompanied by an affirmation by a qualified person, as defined in Section 4C of this Act, on a form provided by the Secretary of State, that the applicant is currently homeless as defined in Section 1A of this Act.\nFor the application for the first Illinois Identification Card of a youth for whom the Department of Children and Family Services is legally responsible or a foster child to be issued at no fee, the youth must submit, along with the application, an affirmation by his or her court appointed attorney or an employee of the Department of Children and Family Services on a form provided by the Secretary of State, that the person is a youth for whom the Department of Children and Family Services is legally responsible or a foster child.\nThe fee for any duplicate identification card shall be waived for any person who presents the Secretary of State's Office with a police report showing that his or her identification card was stolen.\nThe fee for any duplicate identification card shall be waived for any person age 60 or older whose identification card has been lost or stolen.\nAs used in this Section, \"active-duty member of the United States Armed Forces\" means a member of the Armed Services or Reserve Forces of the United States or a member of the Illinois National Guard who is called to active duty pursuant to an executive order of the President of the United States, an act of the Congress of the United States, or an order of the Governor.","path":["CHAPTER 15 EXECUTIVE OFFICERS","Illinois Identification Card Act."],"source_url":"https://www.ilga.gov/legislation/ILCS/details?ActID=204\u0026ChapterID=4\u0026ChapAct=FullText\u0026Print=True","current_through":"at least Public Act 104-790","vintage":"","retrieved_at":"2026-09-15T04:46:12Z","sha256":"2018ebb46fafdfa84fd3799713046ae97267c00a0eda716d1cff23e9d05e6306","source_id":"us-il","stale":false,"prev":"us-il/15-ilcs-335-11a","next":"us-il/15-ilcs-335-12.5"},"notice":"GroundRules: Original legal text.
