{"data":{"id":"us-il/210-ilcs-45-2-207","jurisdiction":"us-il","citation":"210 ILCS 45/2-207","heading":"","body":"(a) Each year the Department shall publish a Directory for each public health region listing facilities to be made available to the public and be available at all Department offices. The Department may charge a fee for the Directory. The Directory shall contain, at a minimum, the following information:\n(1) The name and address of the facility;\n(2) The number and type of licensed beds;\n(3) The name of the cooperating hospital, if any;\n(4) The name of the administrator;\n(5) The facility telephone number; and\n(6) Membership in a provider association and accreditation by any such organization.\n(b) Detailed information concerning basic costs for care and operating policies shall be available to the public upon request at each facility.
