{"data":{"id":"us-il/210-ilcs-49-4-104","jurisdiction":"us-il","citation":"210 ILCS 49/4-104","heading":"Provisional licensure requirements.","body":"Rules governing the provisional license and licensing process shall address, at a minimum, the following provisions:\n(1) mandatory community agency linkage;\n(2) discharge and transition planning;\n(3) non-residential triage centers and stabilization center requirements;\n(4) crisis stabilization;\n(5) transitional living units;\n(6) recovery and rehabilitation supports;\n(7) therapeutic activity and leisure training program;\n(8) admission policies;\n(9) consumer admission and assessment requirements;\n(10) screening and consumer background checks, consistent with Section 1-114.01, subsections (b) and (c) of Section 2-201.5, and Section 2-201.6 of the Nursing Home Care Act;\n(11) consumer records;\n(12) informed consent;\n(13) individualized treatment plan;\n(14) consumer rights and confidentiality;\n(15) safeguard of consumer funds;\n(16) restraints and therapeutic separation;\n(17) employee personnel policies and records;\n(18) employee health evaluation;\n(19) health care worker background check, consistent with the Health Care Worker Background Check Act;\n(20) required professional job positions;\n(21) consultation and training;\n(22) quality assessment and performance improvement;\n(23) consumer information;\n(24) reporting of unusual occurrences;\n(25) abuse and reporting to local law enforcement;\n(26) fire safety and disaster preparedness;\n(27) required support services, including, but not limited to, physician, health, pharmaceutical, infection control, dietetic, dental, and environmental;\n(28) enhanced services requests and program flexibility requests;\n(29) participation in a managed care entity, a coordinated care entity, or an accountable care entity; and\n(30) appropriate fines and sanctions associated with violations of laws, rules, or regulations.","path":["CHAPTER 210 HEALTH FACILITIES AND REGULATION","Specialized Mental Health Rehabilitation Act of 2013."],"source_url":"https://www.ilga.gov/legislation/ILCS/details?ActID=3500\u0026ChapterID=21\u0026ChapAct=FullText\u0026Print=True","current_through":"at least Public Act 104-790","vintage":"","retrieved_at":"2026-09-15T04:46:29Z","sha256":"3e235ac137c7f1000d9580606e01799fc362c735a7e1a6bd14fe9ed7f1cf7705","source_id":"us-il","stale":false,"prev":"us-il/210-ilcs-49-4-103","next":"us-il/210-ilcs-49-4-104.5"},"notice":"GroundRules: Original legal text.
