{"data":{"id":"us-il/215-ilcs-125-4-14","jurisdiction":"us-il","citation":"215 ILCS 125/4-14","heading":"Evidence of Coverage.","body":"(a) Every subscriber shall be issued an evidence of coverage, which shall contain a clear and complete statement of:\n(1) The health services to which each enrollee is entitled;\n(2) Eligibility requirements indicating the conditions which must be met to enroll in a Health Care Plan;\n(3) Any limitation of the services, kinds of services or benefits to be provided, and exclusions, including any reasonable deductibles, copayments, or other charges;\n(4) The terms or conditions upon which coverage may be cancelled or otherwise terminated;\n(5) Where and in what manner information is available as to where and how services may be obtained; and\n(6) The method for resolving complaints.\n(b) Any amendment to the evidence of coverage may be provided to the subscriber in a separate document.","path":["CHAPTER 215 INSURANCE","Health Maintenance Organization Act."],"source_url":"https://www.ilga.gov/legislation/ILCS/details?ActID=1263\u0026ChapterID=22\u0026ChapAct=FullText\u0026Print=True","current_through":"at least Public Act 104-790","vintage":"","retrieved_at":"2026-09-15T04:46:30Z","sha256":"313049e49f4ddf874bf64f2bb366d2080df7f9d2dd4d45ae978d7d9579b32a02","source_id":"us-il","stale":false,"prev":"us-il/215-ilcs-125-4-13","next":"us-il/215-ilcs-125-4-15"},"notice":"GroundRules: Original legal text.
