The notice shall be in large type and shall be in a format substantially similar to the following:\nIN THE CIRCUIT COURT OF THE ... JUDICIAL CIRCUIT OF ILLINOIS\n... COUNTY\nIN RE THE ESTATE OF )\n)\n....................., ) CASE NO. ....\na Person with a Disability, )\nNOTICE TO WARD OF RIGHT TO SEEK MODIFICATION\n[Insert name] was appointed your Guardian of the Person on [insert date].\n[Insert name] was appointed your Guardian of the Estate on [insert date].\nYou have the right to ask the court to dismiss this guardianship, to revoke the power of this guardian to act for you, or to modify the duties of any such guardian.\nYou, or someone on your behalf, can make this request, even by an informal letter, a telephone call, or a visit to the court. You should send your letter to the court at the following address; [insert name of judge and mailing address of courthouse].\nThe court may appoint a Guardian ad Litem to investigate and report to the court.
