As used in this chapter, \"declarant\" means a person:\n(1) who has executed an out of hospital DNR declaration under section 11(a) of this chapter;\n(2) for whom a representative has executed an out of hospital DNR declaration under section 11(b) of this chapter; or\n(3) for whom a proxy has executed an out of hospital DNR declaration under section 11(c) of this chapter;\nand for whom an out of hospital DNR order has been written under section 12 of this chapter.","path":["TITLE 16.
