If a certificate of birth is issued from the record described in section 16 of this chapter, the certificate:\n(1) must include:\n(A) the full name of the child;\n(B) the date and place of birth;\n(C) the name of the father;\n(D) the maiden name of the mother; and\n(E) other data required;\n(2) must be uniform in color, size, and content with all other certificates; and\n(3) may not refer to the fact that the child was born out of wedlock.","path":["TITLE 16.
