(a) The department shall place an approved eligible program provider that does not meet:\n(1) the minimum program performance standards described in section 16 of this chapter; or\n(2) any other requirements under this chapter;\non probationary status for the remainder of the applicable state fiscal year.\n(b) If an approved eligible program provider is placed on probationary status for two (2) consecutive years, the:\n(1) eligible program provider is no longer approved to participate in the program; and\n(2) department shall remove the eligible program provider from the approved eligible program provider list.","path":["TITLE 22.
