As used in this chapter, \"utilization review entity\" means an individual or entity that performs prior authorization for one (1) or more of the following:\n(1) An employer who employs a covered individual.\n(2) A health plan.\n(3) A preferred provider organization.\n(4) Any other individual or entity that:\n(A) provides;\n(B) offers to provide; or\n(C) administers;\nhospital, outpatient, medical, prescription drug, or other health benefits to a covered individual.","path":["TITLE 27.
