If the department receives information under section 4 of this chapter from a health care provider or health plan, the department shall keep the following information of the individual who submitted the information confidential:\n(1) Name.\n(2) Address.\n(3) Telephone number.\n(4) Electronic mail address.\n(5) Personal health information.\n(6) Any other information that could identify the individual.","path":["TITLE 27.
