As used in this chapter, \"health plan\" means the following:\n(1) A policy of accident and sickness insurance (as defined in IC 27-8-5-1).\n(2) An individual contract (as defined in IC 27-13-1-21) or a group contract (as defined in IC 27-13-1-16) with a health maintenance organization (as defined in IC 27-13-1-19).\n(3) A preferred provider plan subject to IC 27-8-11 under which dental services are provided.","path":["TITLE 27.
