Each health maintenance organization subject to this article shall pay to the commissioner for deposit into the department of insurance fund established by IC 27-1-3-28 the following fees:\n(1) Three hundred fifty dollars ($350) for filing:\n(A) an application for a certificate of authority; or\n(B) an application for an amendment to a certificate of authority.\n(2) Fifty dollars ($50) for filing each annual report.","path":["TITLE 27.
