As used in this chapter, \"eligible person\" means either of the following:\n(1) An individual with:\n(A) a serious and persistent mental illness;\n(B) a chronic chemical addiction; or\n(C) a serious and persistent mental illness with a co-occurring chronic chemical addiction;\nresulting in a housing crisis for the individual.\n(2) An individual:\n(A) with:\n(i) a serious and persistent mental illness;\n(ii) a chronic chemical addiction; or\n(iii) a serious and persistent mental illness with a co-occurring chronic chemical addiction; and\n(B) who:\n(i) is exiting a residential treatment program or is living in an institution or other restrictive setting; and\n(ii) could, with stable and affordable housing, along with community and home based supports, live independently in the community.","path":["TITLE 5.
