(a) The bureau shall issue a temporary placard to an individual who is certified by:\n(1) a health care provider listed in section 4(b) of this chapter as having:\n(A) a temporary physical disability that requires the temporary use of a wheelchair, a walker, braces, or crutches;\n(B) temporarily lost the use of one (1) or both legs; or\n(C) a temporary and severe restriction in mobility due to a pulmonary or cardiovascular disability, an arthritic condition, or an orthopedic or neurological impairment; or\n(2) an optometrist or ophthalmologist who has a valid unrestricted license to practice optometry or ophthalmology in Indiana to be temporarily:\n(A) blind (as defined in IC 12-7-2.1-38(2)); or\n(B) visually impaired (as defined in IC 12-7-2.1-349).\n(b) A certification under this section must:\n(1) be in a manner and form prescribed by the bureau; and\n(2) state the expected duration, including an end date, of the condition on which the certification is based.\n(c) A temporary placard issued under this section expires on the earlier of the following:\n(1) One (1) year after the date on which the placard is issued.\n(2) The end date set forth in the certification under subsection (b).","path":["TITLE 9.
