{"data":{"id":"us-ks/k.s.a.-40-2130","jurisdiction":"us-ks","citation":"K.S.A. 40-2130","heading":"Same; form directing withholding or withdrawal of life-sustaining procedures to be provided applicants for coverage; retention upon execution.","body":"The association or a member insurer thereof shall provide every applicant for health coverage under the provisions of this act with a form for making a declaration directing the withholding or withdrawal of life-sustaining procedures in a terminal condition in substantial conformance with subsection (c) of K.S.A. 65-28,103, and amendments thereto.
