{"data":{"id":"us-ky/krs-14a.2-050","jurisdiction":"us-ky","citation":"KRS 14A.2-050","heading":"Forms.","body":"(1) The Secretary of State shall prescribe and furnish on request forms for:\n(a) An application for a certificate of existence;\n(b) An application for a certificate of authority;\n(c) An amended application for a certificate of authority;\n(d) A certificate of withdrawal;\n(e) A change of registered office, registered agent, or both;\n(f) A change of principal address;\n(g) The resignation of the registered agent, the registered office, or both;\n(h) An application for a reserved name;\n(i) The renewal of a reserved name;\n(j) The transfer of a reserved name;\n(k) Name registration;\n(l) The annual report; and\n(m) An amendment to the annual report.\n(2) The use of the forms referred to in paragraphs (e), (f), (g), (h), (i), (k), and (l) of\nsubsection (1) of this section shall be mandatory. If the Secretary of State so\nrequires, the use of some or all of the other forms listed in subsection (1) shall be\nmandatory.\n(3) The Secretary of State may prescribe and furnish on request forms for other\ndocuments required or permitted to be filed by this chapter, but their use shall not\nbe mandatory.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=39655","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:48:32Z","sha256":"730e8263bb4c3eb96162af4722d0174b7294ebacd0b8777aa1a50a2196a86784","source_id":"us-ky","stale":false,"prev":"us-ky/krs-14a.2-040","next":"us-ky/krs-14a.2-060"},"notice":"GroundRules: Original legal text.
