{"data":{"id":"us-ky/krs-158.137","jurisdiction":"us-ky","citation":"KRS 158.137","heading":"Educational passports for state agency children.","body":"(1) As used in this section:\n(a) \"State agency child\" or \"state agency children\" means \"state agency children\"\ndefined in KRS 158.135;\n(b) \"School or educational facility\" means any public school, private school, day\ntreatment center, or any other public or private entity that provides educational\nservices to state agency children; and\n(c) \"Educational passport\" means a standard form completed by a school or\neducational facility which a state agency child is leaving which provides a\nreceiving school or facility with basic demographic and academic information\nabout the state agency child.\n(2) When the placement of a state agency child is changed and the state agency child\nmust transfer from one school or educational facility to a different school or\neducational facility, the school or educational facility that the state agency child is\nleaving shall, within two (2) days of the state agency child leaving, prepare an\neducational passport for the child, which shall be delivered to the Cabinet for Health\nand Family Services or the Department of Juvenile Justice. The Cabinet for Health\nand Family Services or the Department of Juvenile Justice shall, within two (2) days\nof enrolling a state agency child in a new school or educational facility, present the\neducational passport to the receiving school or educational facility.\n(3) A standard educational passport form shall be developed by the Kentucky\nDepartment of Education in consultation with the Cabinet for Health and Family\nServices and the Department of Juvenile Justic e.
