The application shall be on forms prepared and furnished by the\ncommission.\n(3) Every license issued to a CDL driver training instructor shall expire on June 30\nfollowing the date of the issuance unless revoked or canceled by the commission.\n(4) Before July 1 of each year, every CDL driver training instructor shall apply for\nrenewal of his or her license.
