{"data":{"id":"us-ky/krs-198b.4011","jurisdiction":"us-ky","citation":"KRS 198B.4011","heading":"Application for licensure as elevator contractor -- Contents -- Eligibility","body":"requirements.\n(1) An application for licensure as an elevator contractor shall contain the following:\n(a) For an applicant who is a person or sole proprietor:\n1. Name;\n2. Residence; and\n3. Business address of the applicant;\n(b) For an applicant that is a partnership:\n1. Names of the partners;\n2. Residential addresses of the partners; and\n3. Business address of the partnership;\n(c) For an applicant that is a domestic corporation:\n1. Name of the principal corporate officer;\n2. Residential address of the principal corporate officer; and\n3. Business address of the corporation;\n(d) For an applicant that is a corporation other than a domestic corporation:\n1. Name of the corporation's agent who is authorized to accept service of\nprocess and official notices on behalf of the corporation in the state; and\n2. Physical address of the corporation's authorized agent;\n(e) The number of years the applicant has engaged in the business of installing,\nmaintaining, or servicing elevators or fixed guideway systems;\n(f) The approximate number of persons, if any, to be employed by the elevator\ncontractor applicant;\n(g) Proof that he or she has complied with workers' compensati on and\nunemployment insurance laws;\n(h) Evidence that the applicant is covered by general liability, personal injury, and\nproperty damage insurance;\n(i) Criminal record of convictions, if any, as verified through a criminal\nbackground check conducted by th e Department of Kentucky State Police.\nThe cost of the background check shall be paid by the applicant; and\n(j) Upon request by the department, supplemental documentation of information\nrequired by this section.\n(2) An applicant for an elevator contractor license shall:\n(a) Employ at least one (1) licensed elevator mechanic who performs the work\ndescribed in KRS 198B.4003; and\n(b) Have proof of compliance with insurance requirements of KRS 198B.4027.","path":["KRS Chapter 198B"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=36591","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:51:51Z","sha256":"71527f0a5b278a1ea89be42e65c4fe939ece3f56257dcf19102bca7ff4ebedae","source_id":"us-ky","stale":false,"prev":"us-ky/krs-198b.4009","next":"us-ky/krs-198b.4013"},"notice":"GroundRules: Original legal text.
