{"data":{"id":"us-ky/krs-213.121","jurisdiction":"us-ky","citation":"KRS 213.121","heading":"Amendment of certificate or report.","body":"(1) A certificate or report registered under this chapter may be amended only in\naccordance with this section and administrative regulations adopted by the cabinet\nto protect the integrity and accuracy of vital records.\n(2) A certificate or report that is amen ded under this section shall be marked\n\"amended,\" except as otherwise provided in this section. The date of amendment\nand a summary description of the evidence submitted in support of the amendment\nshall be endorsed on or made a part of the record. The cab inet shall prescribe by\nadministrative regulation the conditions under which additions or minor corrections\nmay be made to certificates or records within one (1) year after the date of the event\nwithout the certificate or record being marked \"amended.\"\n(3) Upon written request of both parents and receipts of a sworn acknowledgment of\npaternity signed by both parents of a child born to an unmarried woman, the state\nregistrar shall amend the certificate of birth to show the paternity, if paternity is not\nalready shown on the certificate of birth. The certificate shall not be marked\n\"amended.\"\n(4) Upon receipt of a certified copy of an order of a court changing the name of a\nperson born in the Commonwealth and upon request of the person or the person's\nparents, guardian, or legal representative, the state registrar shall amend the\ncertificate of birth to show the new name.\n(5) Upon receipt of a sworn statement by a licensed physician indicating that the gender\nof an individual born in the Commonwealth has been changed by surgical procedure\nand a certified copy of an order of a court of competent jurisdiction changing that\nindividual's name, the certificate of birth of the individual shall be amended as\nprescribed by regulation to reflect the change.","path":["KRS Chapter 213"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=8741","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:52:17Z","sha256":"8eb4f08fe45e54dc7758ead65bd31ad5b1ae49e6c9339fc783189ebacf01dd65","source_id":"us-ky","stale":false,"prev":"us-ky/krs-213.120","next":"us-ky/krs-213.122"},"notice":"GroundRules: Original legal text.
