{"data":{"id":"us-ky/krs-216b.235","jurisdiction":"us-ky","citation":"KRS 216B.235","heading":"Consultation with designated lay caregiver and development of discharge","body":"plan -- Administrative regulations.\n(1) As soon as practicable a hospital shall consult with a designated lay caregiver\nregarding the patient's after -care needs. If the hosp ital is unable to contact the\ndesignated lay caregiver, the lack of contact shall not interfere with, delay, or\notherwise affect an appropriate discharge of the patient.\n(2) A discharge plan shall include:\n(a) The name and contact information of the designated lay caregiver;\n(b) A description of after -care tasks the patient may perform at the patient's\nresidence; and\n(c) Contact information for health care, community, and long -term care resources\nand supports that may be available and appropriate to assist in implementing\nthe patient's discharge plan.\n(3) The purpose of the hospital's consultation with a patient's lay caregiver as described\nin this section is to assist the lay caregiver in preparing for the patient's after -care\nneeds, which may include demon strations of after -care tasks and an opportunity to\nask questions.
