{"data":{"id":"us-ky/krs-281a.140","jurisdiction":"us-ky","citation":"KRS 281A.140","heading":"Form of application -- Change of address -- Lost or destroyed license --","body":"Penalty.\n(1) The application for a commercial driver's license or commercial learner's permit\nshall include the following information:\n(a) The applicant's full legal name, any nicknames or other names by which he or\nshe is known, and present Kentucky resident address. If the applicant's\nmailing address is different from the resident address, the mailing address\nshall also be included;\n(b) A physical description of the applicant including sex, height, weight, eye\ncolor, and race;\n(c) The applicant's date of birth;\n(d) The applicant's Social Security number;\n(e) The applicant's signature;\n(f) Certifications including those required by 49 C.F.R. secs. 383.71, 383.73, and\n384.206, as adopted by the cabinet;\n(g) A consent to release driving record information;\n(h) A valid Class D operator's license issued pursuant to KRS 186.4102 and\n186.412;\n(i) A birth certificate if the applicant does not hold a valid operator's license at\nthe time of application;\n(j) The name of every jurisdiction in which the applicant has previously been\nlicensed to drive any type of motor vehicle during the ten (10) year period\nimmediately preceding the date of the application; and\n(k) Any other information required by the cabinet.\n(2) The cabinet or state police may require any other information needed in order to\nprocess the application.\n(3) When the holder of a commercial driver's license changes his or her name or\nresidence, the information shall be reported to the cabinet within  ten (10) days. The\nholder of a Class A, B, or C license shall make an application for a duplicate license\nwithin thirty (30) days of changing his or her name or address.\n(4) Any person whose commercial driver's license has been legitimately lost or\ndestroyed shall make an application for a duplicate. A person applying for a\nduplicate within the time period for which the original license was issued shall\napply to the Transportation Cabinet. The person shall provide the cabinet with proof\nof the person's ide ntity and a notarized affidavit with a raised seal explaining in\ndetail the loss or destruction of the original license.\n(5) A person who is a resident of this state shall not drive a commercial motor vehicle\nunder the authority of a commercial driver's license issued by another jurisdiction.\n(6) Any person who knowingly falsifies information or certifications required to obtain\na commercial driver's license, a commercial driver's license permit, or a duplicate\ncommercial driver's license subsequent to an a dministrative hearing conducted in\naccordance with KRS 186.570, shall be subject to suspension, revocation, or\ncancellation of his or her commercial driver's license for a period of at least sixty\n(60) consecutive days.","path":["KRS Chapter 281A"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=55028","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:53:35Z","sha256":"ed28bdf801e1e42c0f23bf4bbd654f198466ce7a9288a4d49babb66ae7bd622c","source_id":"us-ky","stale":false,"prev":"us-ky/krs-281a.130","next":"us-ky/krs-281a.150"},"notice":"GroundRules: Original legal text.
