{"data":{"id":"us-ky/krs-286.4-430","jurisdiction":"us-ky","citation":"KRS 286.4-430","heading":"Form of application -- Contents.","body":"(1) Each application for a license under this subtitle shall be made in writing, under\noath or affirmation, in a form the commissioner prescribes.\n(2) Each application shall contain the following information:\n(a) In the case of an applicant that is a natural p erson, the name, electronic mail\naddress, and physical address of the residence and place of business of both\nthe applicant and, if applicable, the managing principal;\n(b) In the case of an applicant that is a partnership, limited liability company, or\nassociation:\n1. Names, electronic mail addresses, and physical addresses of every\nmember and managing principal; and\n2. The physical address where the business is to be conducted;\n(c) In the case of an applicant that is a corporation:\n1. The names, electronic  mail addresses, and physical addresses of the\nprincipal officers, directors, and managing principal; and\n2.
