{"data":{"id":"us-ky/krs-286.9-104","jurisdiction":"us-ky","citation":"KRS 286.9-104","heading":"Annual reports filed by licensees with commissioner.","body":"(1) Each licensee shall file an annual report with the commissioner by March 1 of each\nyear, containing the following information:\n(a) The names and addresses of each person owning a controlling interest in each\nlicense;\n(b) The location of all places of busin ess operated by the licensee and the nature\nof the business conducted at each location;\n(c) The names and addresses of all affiliated entities regulated under this subtitle\nand doing business in this state;\n(d) Balance sheets, statement of income and expen ses, and other statistical\ninformation as may be reasonably required by the commissioner, consistent\nwith generally accepted accounting practices, for the purpose of determining\nthe general results of operations under this subtitle; and\n(e) If the licensee  is a corporation, the names and addressees of its principal\nofficers and directors; or if the licensee is a partnership, the names and\naddresses of the partners; or if the licensee is a limited liability company, the\nnames and addresses of the board of directors of the limited liability company.\n(2) If the licensee holds two (2) or more licenses or is affiliated with other licensees, a\ncomposite report may be filed.\n(3) All reports shall be filed in a form as may reasonably be required by the\ncommissioner and shall be sworn to by a responsible officer of the licensee.\n(4) The information submitted by licensees under this section shall be held in\nconfidence by the department and the commissioner.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=14915","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:57:15Z","sha256":"3bd890c347f502e36b7d20f007080f9e37e4b9713177be918573042d1b8a987a","source_id":"us-ky","stale":false,"prev":"us-ky/krs-286.9-102","next":"us-ky/krs-286.9-105"},"notice":"GroundRules: Original legal text.
