Unless either\nso attached and accompanying the policy, or printed on the face or reverse side thereof,\nthe same shall not be received as evidence in any action for the recovery of benefits\nprovided by the policy, and shall not be considered a part of the policy or of the contract\nbetween the parties, but nothing in this section is intended to apply to evidence used in\nthe reinstatement of a policy.
