{"data":{"id":"us-ky/krs-29a.070","jurisdiction":"us-ky","citation":"KRS 29A.070","heading":"Juror qualification forms.","body":"(1) The Chief Circuit Judge shall cause to be mailed or delivered with the summons to\neach juror a juror qualification form accompanied by instructions to fill out and\nreturn the form by mail or hand delivery to the clerk within five (5) days after its\nreceipt, or to be completed as otherwise directed. The juror qualification form shall\nbe provided by the Administrative Office of the Courts and subject to approval by\nthe Chief Justice of the Supreme Court.\n(2) The juror qualification form shall contain the pros pective juror's signed declaration\nthat his responses are true to the best of his knowledge and his acknowledgment that\na willful misrepresentation of a material fact may be punished by a fine or by\nimprisonment, or both. Notarization of the juror qualific ation form shall not be\nrequired.\n(3) If the prospective juror is unable to fill out the form, another person may do it for\nhim and shall indicate that he has done so and the reason therefor.\n(4) Any prospective juror who fails to return a properly complet ed juror qualification\nform as instructed may be directed by the Chief Circuit Judge to appear forthwith to\nfill out a juror qualification form. At the time of his appearance for jury service, any\nprospective juror may be questioned by the judge or his des ignee, but only with\nregard to his responses to questions contained on the form and grounds for his\nexcuse or disqualification. Any information thus acquired shall be noted on the\njuror qualification form.\n(5) A prospective juror who fails to appear as dir ected by the judge pursuant to\nsubsection (4) of this section shall be ordered to appear and show cause for his\nfailure to appear as directed. If the prospective juror fails to appear pursuant to the\njudge's order or fails to show good cause for his failur e to appear as directed by the\njudge, he may be punished for contempt.\n(6) No person shall willfully misrepresent a material fact on a juror qualification form.\n(7) The contents of juror qualification forms shall be made available to parties or their\nattorneys of record unless the court determines in any instance in the interest of\njustice that this information shall be kept confidential or its use limited in whole or\nin part.","path":["KRS Chapter 29A"],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=21124","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:48:49Z","sha256":"cb04e1731e163de4e774523052134d97591c3aa9391679b79721befed7f27399","source_id":"us-ky","stale":false,"prev":"us-ky/krs-29a.060","next":"us-ky/krs-29a.080"},"notice":"GroundRules: Original legal text.
