{"data":{"id":"us-ky/krs-304.14-135","jurisdiction":"us-ky","citation":"KRS 304.14-135","heading":"Uniform health insurance claim forms.","body":"(1) The commissioner shall prescribe the following uniform health insurance claim\nforms which shall be used by all insurers transacting health insurance in this state\nand by all state agencies that require health insurance claim forms for their records\nas the sole instrument for reimbursement:\n(a) The uniform health insurance claim form for an institutional provider shall\nconsist of the UB -92 data set or its successor submitted on the designated\npaper or electronic format as adopted by the National Uniform Bill ing\nCommittee;\n(b) The uniform health insurance claim form for a dentist shall consist of a data\nset and form approved by the American Dental Association;\n(c) The uniform health insurance claim form for all other health care providers\nshall consist of the HCFA 1500 data set or its successor submitted on the\ndesignated paper or electronic format as adopted by the National Uniform\nClaim Committee; and\n(d) A clean claim for pharmacists shall consist of:\n1. For prescription drug claims, a universal claim form a nd data set\napproved by the National Council for Prescription Drug Programs; and\n2. For all other claims for services or procedures that are within the scope\nof the practice of pharmacy, as defined in KRS 315.010, a 1500 Health\nInsurance Claim Form or its successor submitted on the designated\npaper or electronic format as adopted by the National Uniform Claim\nCommittee.\n(2) An insurer shall not require a provider to:\n(a) Use a claim form that is different than the uniform claim form for the provider\ntype as set out in subsection (1) of this section;\n(b) Modify the uniform claims form or its content; or\n(c) Submit additional claims forms.","path":[],"source_url":"https://apps.legislature.ky.gov/law/statutes/statute.aspx?id=50952","current_through":"Includes enactments through the 2026 Regular Session","vintage":"09/05/2026","retrieved_at":"2026-09-05T20:57:43Z","sha256":"88fde5f5e4ecb3d1a001f35ff4cf779938972705bf549b0f61c613a4139d25c7","source_id":"us-ky","stale":false,"prev":"us-ky/krs-304.14-130","next":"us-ky/krs-304.14-140"},"notice":"GroundRules: Original legal text.
